Conversion Options |
Convertible to Age 75 without additional evidence of
insurability to any form of INDIVIDUAL permanent insurance
coverage then offered by the Company for an amount up to the
face amount of the policy. This option is available through
the policy anniversary nearest the insured's age 75. Premium
rates on the new policy will generally be based on the
insured's attained age at the time of conversion unless an
original issue age exchange is requested within the first
five years.
The risk class under the new policy will be substantially
comparable to the class assigned to the Term policy being
exchanged. The policyowner can, in accordance with the terms
of the policy, execute either an original issue age or an
attained age conversion within the first five years. After
the fifth year, only attained age conversions will be
allowed. |

Special Product Features |
Disability Waiver of Premium Provides that, in the event
of total disability prior to age 60, the Company will waive
the premium for the base plan, benefit premiums and any
other rider premiums.
Available for Issue Ages 18 - 55. May be rated
independent of the base policy at 150% or 200% of standard
rate. Benefit terminates at Age 60. Premiums are level
during the level period and increasing thereafter.
Accidental Death Benefit (ADB) Provides a specified
death benefit amount in addition to the policy's basic death
benefit if the insured dies as a result of an accident, as
defined in the policy.
Available for Issue Ages 20 - 60. May be rated
independent of the base policy at 150% or 200% of the
standard rate. Maximum - Lesser of $250,000 or 2 times
the base policy face amount. Benefit terminates at Age 70.
Premiums vary and generally increase annually.
Living Benefits Rider (LBR) This rider permits the
policyowner to access a portion of the life insurance
proceeds during the insured's lifetime in the event that the
insured is diagnosed as terminally ill (life expectancy of
12 months or less; in the state of Massachusetts, a life
expectancy of 24 months or less).The amount available varies
by state, but may be as much as 75% (not to exceed
$250,000)of the life insurance proceeds payable upon death
Available for Issue Ages 18 - 65. Available for all Risk
Classes including Rated Classes.
NOTE: Receipt of benefits may be taxable and may affect
eligibility for public assistance programs. |

Tobacco Definition |
Preferred Plus Nonsmoker An individual who qualifies
for Preferred Plus and has not used tobacco or nicotine
products in the last 5 years.
Exception for Occasional Cigar Smokers: Occasional cigar
smokers, those who will attest on the application to smoking
12 or fewer cigars per year, can qualify for Preferred Plus,
provided their urinalysis is negative for cotinine.
Preferred Nonsmoker An individual who qualifies for
Preferred and has not used tobacco or nicotine products in
the last 36 months.
Exception for Occasional Cigar Smokers: Those who will
attest on the application to smoking 4 or fewer cigars per
month can qualify for Preferred, provided their urinalysis
is negative for cotinine.
Standard Nonsmoker An individual who qualifies for
Standard has not used any nicotine products during the year
preceding the application
Exception for Occasional Cigar Smokers: Those who will
attest on the application to smoking 4 or fewer cigars per
month can qualify for Preferred, provided their urinalysis
is negative for cotinine.
Preferred Smoker An individual who qualifies for the
Preferred class and has smoked cigarettes within the past 12
months.
Standard Smoker A cigarette smoker who does not qualify
for the Preferred Smoker class and does not have a ratable
impairment. |
Special Underwriting Features |
Preferred Plus Nonsmoker To qualify for the Premier
LT Extra Preferred Plus class, the proposed insured must
meet the following criteria. Much of this data will be
available at the point of sale and should serve as a
guideline in determining whether to quote the Preferred Plus
rate.
No use of tobacco or nicotine products in the last 5 years.
Exception for Occasional Cigar Smokers: Occasional cigar
smokers – those who will attest on the application to
smoking 12 or fewer cigars per year – can qualify for
Preferred Plus, provided their urinalysis is negative for
cotinine.
Family History: No deaths or disease of a natural parent or
sibling prior to age 60 due to cancer, stroke, heart
problems or diabetes mellitus.
Blood Pressure (Without Treatment) 130/80 for ages 0 – 55
140/85 for ages 56+ Cholesterol (Treatment ok if
well-controlled) Less than or equal to 215
HDL/Cholesterol Ratio Less than or equal to 5.0
No medical history of cancer (except superficial skin),
coronary artery diseases, diabetes mellitus, ratable medical
history, counseling or treatment for alcohol or drug abuse.
Driving Record: No more than three moving violations in the
past three years; no DWI in the past ten years.
No private pilots (except commercial airlines), no hazardous
avocations, no hazardous occupations.
Preferred Nonsmoker
To qualify for the Premier LT Extra Preferred class, the
proposed insured must meet the following criteria. Much of
this data will be available at the point of sale and should
serve as a guideline in determining whether to quote the
Preferred rate.
No use of tobacco or nicotine products in the last 36
months. (NOTE: To qualify for the standard nonsmoker rates,
the applicant must not have smoked cigarettes for the past
12 months. Cigar and pipe smokers typically qualify for
nonsmoker rates.)
Exception for Occasional Cigar Smokers: Those who will
attest on the application to smoking 4 or fewer cigars per
month can qualify for Preferred, provided their urinalysis
is negative for cotinine.
Family History: No more than one death of a natural parent
or sibling prior to age 60 due to cancer, stroke, heart
problems or diabetes mellitus. If parents or siblings are
living, then no more than one can have a history of cancer,
stroke, heart problems or diabetes mellitus prior to age 60.
Blood Pressure (Treatment ok if well-controlled) 135/85
for ages 0-55 140/90 for ages 56+ Total Cholesterol
(Treatment ok if well-controlled) Less than or equal to
230 HDL/Cholesterol Ratio Less than or equal to 5.5
