Type |
Prelude Plus GP term series rates are level during the
guarantee period. After the guarantee period ends, premiums
increase annually on a current and guaranteed basis to age
95. Premiums are payable to age 95. | |
Conversion Options |
Prelude Plus GP series policies may be converted at any time
during the policy years shown below or through the insured’s
age 69, whichever comes first. Conversion will not be
permitted while the policy is on a disability waiver claim.
GP-10: First 10 policy years GP-15: First 15 policy years
GP-20: First 20 policy years GP-30: First 30 policy years
The Prelude Plus GP series policy may be converted to any
permanent single life product written by Jefferson Pilot
Financial Insurance Company or Jefferson-Pilot Life
Insurance Company available on the date of conversion. No
proof of insurability is required if the amount of insurance
on the new policy is no greater than that of the old policy,
and the new policy does not contain any additional riders.
Partial conversions may be done as long as the amount of
insurance remaining meets the minimum issue amount
requirement of $500,000. There are no conversion credits.
Proof of insurability and company approval will be required
if the new policy:
contains benefits or riders, even if they were on the

involves insurance on another life
includes an increase in the amount of life insurance

Policy Fee |
A non-commissionable policy fee of $75 is charged each year.
The policy fee will be reduced to $37.50 when underwritten
concurrently with selected Jefferson Pilot Financial
Insurance Company or Jefferson-Pilot Life Insurance Company
products.
For spouses or business partners/owners who apply for
Prelude Plus GP insurance together, the $75 annual term
policy fee will be reduced to $37.50 on each policy. To
qualify for this rate, applications must be received, issued
and paid together. |
Special Product Features |
Accelerated Benefit:
This rider allows a policyowner to receive up to 50 percent
of the policy death benefit within six months of expected
death of the insured (subject to a $250,000 maximum). There
is no premium charge for this rider. (Please note that this
benefit is for terminal illness only. A critical illness or
nursing home benefit is not available.) The owner must apply
for this rider and request the benefit. An administrative
charge of $300 and an actuarial discount are deducted from
the benefit amount.
Children’s Insurance:
This rider provides level term insurance for each child. All
children, including stepchildren and legally adopted
children of the insured who are members of the insured’s
household, at least 15 days old and under the age of 18 on
the day of issue and who are named on the application are
covered.
In addition, children who are born or legally adopted by the
insured after the date of application are covered.
Issue amounts are 1 to 15 units of $1,000 each. The premium
is $5 per unit. Issue ages (of the base insured) are 18-55.
Rider coverage expires on the earlier of the youngest
child's 25th birthday or policy insured's age 65. The
coverage for a child may be converted to a permanent plan of
insurance without evidence of insurability at any time
within 60 days before and 31 days after the coverage expires
for an amount up to five times the amount of the coverage.
Waiver of Premium:
This rider waives the total premium in the event of and
during total disability occurring prior to the policy
anniversary nearest the insured’s 65th birthday. Total
disability must exist for four months before any benefit is
payable.
Issue ages are 18-59. It is available for substandard
policies to Table D. Premiums are payable to the policy
anniversary nearest the insured’s 65th birthday. |

Re-entry |
On the Exchange Date (end of the guaranteed level premium
period of 10, 15, 20 or 30 years) the owner may exchange the
policy for a new policy subject to the following conditions:
The
face amount of the new policy must be equal to or less than
the face amount of the original policy.
The new policy will be on the same plan as the initial
policy.
A new application must be submitted not more than 90 days or
less than 60 days before the Exchange Date.
Satisfactory evidence of insurability must be submitted.
The policy date of the new policy will be the Exchange Date.
The issue age on the new policy will be the insured’s
attained age nearest birthday on the Exchange Date.
The issue age on the new policy cannot exceed the maximum
issue age for the plan.
The premium rates on the new policy will be those then in
use at the time of the exchange.
Any rider or benefit which was on the original policy may be
included on the new policy subject to the rules and premium
rates then in use. | |
Tobacco Definition |
Preferred Plus Non-Tobacco User — Preferred Plus is our
best risk classification and includes those applicants who
have not used tobacco or nicotine in any form in the past 36
months and meet all of the "preferred" and "preferred plus"
criteria.
Preferred Non-Tobacco User — This class is for clients
who enjoy exceptional health and who have not used tobacco
or nicotine in any form in the past 24 months, except for
the occasional cigar user provided the urine specimen is
negative for nicotine, have not used cigarettes in the past
5 years, and meet all of the "preferred" criteria.
Preferred Tobacco User---This class is similar to the
preferred non-tobacco class, but the insured has used
tobacco or nicotine within the past year and/or has a urine
specimen that is positive for nicotine. This includes users
of pipes and/or chewing tobacco.
Standard Non-Tobacco User — This class represents those
insureds who have an average life expectancy, have not used
tobacco or nicotine in any form in the past 12 months and do
not meet the "preferred" criteria. This class includes the
substandard risks for the non-tobacco user classes and those
occasional cigar users who have not used cigarettes in the
past 5 years and who test negative for nicotine in the
urinalysis.
Standard Tobacco User — This class is similar to the
standard non-tobacco class, but the insured has used tobacco
or nicotine within the past year and/or has a urine specimen
that is positive for nicotine. This includes users of pipes
and/or chewing tobacco. This class includes the substandard
risks for the non-tobacco user class. |
Special Underwriting Features |
Preferred Plus Non-Tobacco User Criteria:
The preferred criteria must be met as well as the following
criteria:
*No personal history of cardiovascular disease, diabetes,
and/or cancer (excluding benign skin cancer).
*No use of tobacco or nicotine in any form within the last
36 months.
*No history of alcohol or drug abuse.
*No private aviation or other hazardous activity including
scuba diving.
*No family history of diabetes or coronary artery disease
resulting in death of a parent or sibling prior to age 65.
*No more than one moving violation in the past two years or
two moving violations during the past three years. No
history of a DWI/DUI during the past five years.
*Cholesterol level must be lower than 225 and the
Cholesterol/HDL ratio must be lower than 5.0 at ages less
than 45. Cholesterol level must be lower than 230 and the
Cholesterol/HDL ratio must be lower than 5.5 at ages 45 and
up.
*No treated hypertension (medication) within the past 12
months with a 12 month average of 140/90 or better for all
ages.
Preferred Non-Tobacco User Criteria:
Standard risk with no extra mortality.
*No tobacco or nicotine within the past 24 months. A
nicotine positive HOS will preclude preferred consideration.
The occasional cigar user (using no more than one per week)
may also be considered for preferred provided the urine
specimen is negative for nicotine and they have not used
cigarettes in the past 5 years, and meet all of the other
"preferred" criteria.
*No family history of diabetes or coronary artery disease
resulting in the death of a parent or sibling prior to age
60.
*No private aviation (pilot or crew member of regularly
scheduled commercial airline does not preclude Preferred).
*No ratable hazardous avocations or occupations.
*No history of alcohol or drug abuse in the last 10 years.
*No more than one moving violation in the past year or more
than two driving violations within the previous three years.
No DUI/DWI or license suspension within the past five years.
*Must be a US or Canadian citizen or permanent resident. No
travel to countries where there is a risk of exposure to
terrorism. No travel to a Third World country on a regular
basis.
*Blood pressure readings, treated and untreated, must
average, for the past 12 months: 140/90 or better if less
than age 65; 150/90 or better if age 65 or older
*Cholesterol level must be lower than 230 at ages 45 and
below. Cholesterol level must be less than 245 at ages 45
and up. |
