Preferred Elite Class
(for face amounts $3,000,000):
1. Must be a non-user of tobacco products or nicotine
substitutes for at least the past five years.
2. No occurrence of coronary disease, stroke, invasive
internal cancer or malignant melanoma before the age of 60
in natural parents or the siblings.

4. There must not be any history or current treatment for
elevated blood pressure (hypertension). Current blood
pressure average must not exceed 135/85 for ages 0-60;
145/90 for ages 61+.
5. There must not be any history or current treatment for
elevated cholesterol (hyperlipidemia). Current cholesterol
levels must not exceed 220 mg%; 250 mg% if cholesterol to
HDL ratio is <4. Cholesterol to HDL ratio must not exceed
4.5:1.
6. Must be a standard risk for occupation.
7. No DUIs or reckless driving within last 10 years (maximum
one allowed within lifetime) and no more than one moving
violation within last three years.
8. Must not have participated in a hazardous avocation
within the last two years. Recreational scuba diving up to
50 feet will not disqualify an insured from the Preferred
Elite classification.
9. No flying as pilot or crewmember except on regularly
scheduled U.S. based airline.
10. No active military risk will be acceptable for Preferred
Elite rates.
11. Must have resided in United States for three years and
have no intention of extensive foreign travel to
underdeveloped countries or areas of political instability.
12. Must never have been treated for alcohol or drug abuse.
Preferred Class: 1. Must be a non-user
of all tobacco products or nicotine substitutes for at least
the past three years.
2. There must be no history of death of parent due to
coronary dis-ease, stroke, invasive internal cancer or
malignant melanoma before the age of 60.
3. The insured's weight must be less than the maximum listed
for the corresponding height on the build chart.
4. Individuals with controlled elevated blood pressure will
be accepted as long as the current and historical blood
pressure average over the last two years does not exceed
140/90 for ages 0-60; 150/90 for ages 61+.
5. Individuals with treatment for elevated cholesterol
(hyperlipidemia) will be accepted as long as the current and
historical cholesterol levels over the last two years meet
the following parameters: Must not exceed 250 mg%, or 275
mg% if Cholesterol to HDL ratio is <5.5. Cholesterol to HDL
ratio must not exceed 6:1.
6. Hazardous occupations are not excluded but flat extra fee
may apply.
7. No DUIs or reckless driving within last five years and no
more than two moving violations within last three years.
8. Avocation activities are not excluded but a flat extra
fee may apply.
9. No flying as pilot or crewmember except: (1) on
regularly scheduled U.S. based airline or (2) private pilots
with over 300 hours of experience flying 50-150 hours per
year with IFR.
10. No active military risk below the rank of commissioned
officers will be acceptable for Preferred rates.
Commissioned officers must not be involved in hazardous
military duties, e.g. aviation or special forces.
11. Must have resided in United States for three years and
have no intention of extensive foreign travel to
underdeveloped countries or areas of political instability.
12. Must never have been treated for alcohol or drug abuse.
Standard Select Class: 1. Must be a non-user
of all tobacco products or nicotine substitutes for at least
the past two years.
2. There must be no more than one death of a parent due to
coronary disease, stroke, invasive internal cancer or
malignant melanoma before the age of 60.
3. The insured’s weight must be no more than the maximum
listed for the corresponding height on the build chart.
4. Individuals with controlled elevated blood pressure will
be accepted as long as the current and historical blood
pressure average over the last two years does not exceed
145/90 for ages 0-60; 155/95 for ages 61+.
5. Individuals with treatment for elevated cholesterol
(hyperlipidemia) will be accepted as long as the current and
historical cholesterol levels over the last two years meet
the following parameters: Must not exceed 285 mg%.
Cholesterol to HDL ratio must not exceed 7:1.
6. Hazardous occupations are not excluded but a flat extra
fee may apply.
7. No DUIs or reckless driving within last five years and no
more than three moving violations within the last three
years.
8. Participation in hazardous avocation and scuba diving
within the last two years will not automatically disqualify
the applicant.
9. Private pilots will be considered for Standard Select
rates with the addition of a flat extra fee or Aviation
Exclusion Rider where warranted.
10. Active military personnel who are not commissioned
officers will be considered for Standard Select rates and
may require addition of flat extra rating if involved in
hazardous military duties, e.g. aviation or special forces.
11. Must not have been treated for alcohol or drug abuse
within the last five years.
12. Must have resided in United States for one year and have
no intention of extensive foreign travel to underdeveloped
countries or areas of political instability.
