Chase Insurance - Super-T Product Features
State Approvals Super-T 2003 is available in all states except MS NJ NY OR. MS NJ OR continue to use 2002 rates.
Type 20 and 30-year level premium plans. The level-premium guarantee period is 10 years for both plans.
Conversion Options Prior to the earlier of the 20th policy anniversary or the anniversary nearest the insured’s 70th birthday, a Super-T policy may be converted to an eligible cash value life insurance policy. The new policy must be available through Zurich Life at that time and have a higher minimum-premium rate than the Super-T plan at the time of conversion.

Accelerated Death Benefit Rider (XDB)*
This rider is available to issue ages 1 through 80 without additional
premium cost and is added automatically to the base policy and
companion policy coverages in states where this rider is approved; it
will be added, retroactive to the issue date of the base policy, once
a state does approve it.(See policy form S-9325 for additional details)

Dependent Children’s Rider (DCR)
Person(s) covered: All children
Issue ages: Age 18-60, primary insured; Age 15 days to 18 years, children
Substandard: Through Table H for insured; rated children excluded from coverage
Minimum amount: 5 units ($5,000)
Maximum amount: 20 units ($20,000)
Expires: Earlier of base-plan anniversary nearest insured’s age 65 or as each child reaches age 23
Rate: $8 annually per unit (See policy form S-9308 for additional details)

Waiver of Premium (WP)*
Person covered: Base-plan insured or companion policy insured
Issue ages: 20 through 55
Premiums waived: All premiums, including base plan, riders, policy fee, and substandard extras
Substandard: Through Table D
Minimum amount: Same as base plan
Expiration: Policy anniversary nearest insured’s attained age 60 (but
premiums will continue to be waived if eligible disability occurs
before and continues beyond age 60)
Rate: Multiply total premium (including base plan, sub-standard
extras, other rider premiums, and policy fee) by factor 0.21951 (See policy form S-9335 for additional details)

Re-entry Chase Insurance Super-T plans allow for one re-entry qualification, at the end of the initial selected period (i.e., after 20 or 30 years), as long as the insured is younger at the time of re-entry than the maximum issue age for that plan. Re-entry requires evidence of insurability (at company cost). Insureds who qualify avoid the annually increasing premiums that would otherwise follow their initial level-premium period. Instead, re-entry premiums are the same as those for a newly issued policy at the insured’s age at the time of re-entry and are again level for the same number of years (20 or 30) as the initial period.
Tobacco Definition Non-Tobacco rates apply to Preferred, Select, and Standard if the insured has not used tobacco in any form within the last 36 months prior to application

Premier no tobacco use in the last 60 months.

Occupation/Avocation: No hazardous occupation, sports or hobbies in past 2 yrs or next 2 yrs

Aviation: No flying as pilot or crew member except on regularly scheduled major commercial airline

Driving History: No more than 2 violations in past 3 years. No DUI or Reckless Driving in past 5 years.

Foreign Travel: No travel to underdeveloped countries or areas of political instability within past 2 years or next 2 years.

U.S. Residency: No residence outside U.S. in past 3 yrs.

Preferred Non-Tobacco:
BP Average: Age 0-60: No treatment, 140/90; With treatment, 135/85; Ages 61+: No treatment, 150/90; With treatment, 140/90

Cholesterol: Does not exceed 240/6.0 HDL; Treatment OK

Family History: No death from CAD or colon or prostate cancer (males) and no CAD or breast, ovarian, or colon cancer (females) in parent or sibling prior to age 60

Occupation/Avocation: Flat extra if applicable

Aviation: No flying as pilot or crew member except on regularly scheduled major commercial airline.
